I don’t have my insurance card with me, can I still receive treatment?

We prefer that you have your driver's license and your insurance cards available  before the visit. If you do not, we request cash payment at the time of service. You can submit to your insurance afterwards for reimbursement of your services received.

How much will my copayment be?

Your copayment is typically due at the time of the service. Your co-payment is determined by your particular insurance company. This is a specialty service, and specialty rates will apply. If you are concerned about the amount of your co-payment, please contact your insurance company directly.

What is your new patient procedure?

We ask that you arrive 15 minutes early to fill out the paperwork if you are new. There is not a front office check in desk- please be seated in the waiting area and someone will greet you and get your insurance card and ID and have your paperwork to be completed.
